Private Pay (Out-of-Network*)
Diagnostic Assessment Session (60 min intake + 60 min follow-up): $450
Individual Therapy Session (60 mins): $200
KAP: Ketamine-Assisted Dosing Session (3 hrs): $550
Detailed Letter: $40*
No Show/ Late Cancellation Fee: $100
Out-of-Network: Insurance
Therapy services are considered out-of-network. If you have out-of-network benefits, you may be eligible for a partial reimbursement from your insurance plan. Contact your insurance plan for more information. Services such as Thrizer and Mentaya help streamline the claim submission process for out-of-network claims. No show fees are not reimbursable by insurance. Medicare & Medicaid are not eligible for out-of-network services.
Late arrivals & cancellation policy
Cancellations must be made 24 hours prior to the appointment. A 10 minute late arrival to a session is considered a no show appointment. The $100 no show fee will be automatically charged to the card on file. Cancellations may be made via the client portal.
*Paperwork is only provided for current clients. Fee may be avoided by requesting letters and completing forms together in session
